Pathology Club Annual Report

All active ACVP-Recognized Veterinary Student Pathology Clubs are required to report annually by the published deadline. Submitters are advised to preview all questions by clicking on the numbered page squares at the top of the form or "Next" button so that you may gather all required information before returning later to complete the form in its entirety in one sitting. (When previewing the form, you are advised to not save and continue, and to ignore error messages regarding incomplete fields. The final report cannot be submitted until all required fields are completed.)

Required information includes:
Faculty advisor names and emails
Student officer names and emails
Path club activities for the calendar year (January to December)
Path club travel support and fundraising information
Club member ACVP Annual Meeting participation information
Pathology training plans for final-year members

Information required for ACVP-Recognized Pathology Club Travel Award consideration includes:
Club member Annual Meeting participation specifics
Institutional (student organization or university) tax and bank information

Travel Awards may only be paid to the student organization (pathology club) or university for subsequent distribution to members by that entity. Questions about the payment should be submitted by an ACVP contact form
